GOAL

Prepare individuals to find and sustain permanent housing after discharge, while ensuring each individual is empowered to manage their healthcare needs, provider choices and ability to advocate for themselves.

ELIGIBILITY CRITERIA

KERN COUNTY

  • Admission paper work has to be coordinated with the center from the institution of transfer prior to admission.

  • Residents are authorized by the State to stay months and one admission per lifetime.

​

  • Residents must be able to self mobilize household distances and use restrooms and showers without mechanical assistance.

​

  • Residents should be able to self administer medications, with help of health literacy support staff and ancillary team.

​

  • Medications are stored in locked secure setting and to be received under staff supervision.

​

  • On-site staff supervision is provided 24/7. Nurse Practioner (NP) rounds and Licensed vocational nurse (LVN) assistance.

TULARE COUNTY

  • Admission paper work has to be coordinated with the center from the institution of transfer prior to admission.

​

  • Residents are authorized by the State to stay for up to 30 days, with extensions in 30 day increments. A reassessment is required for each extension. There is a 6 month limit.

​

  • Residents must be able to self mobilize household distances and use restrooms and showers without mechanical assistance.

​

  • Residents should be able to self administer medications, with help of health literacy support staff and ancillary team.

​

  • Medications are stored in locked secure setting and to be received under staff supervision.

​

  • On-site staff supervision is provided 24/7. Licensed vocational nurse (LVN) assistance.
